Hall Usage Agreement

I, representing my organisation, have read and agreed to the Terms and Conditions stated on the Rental of Event Hall document and hereby confirm that I/we shall use the event hall at Nanyang Pho Leng Hui Kuan on level 6 for our event.

A security deposit of $1,000 shall be made payable to Nanyang Pho Leng Hui Kuan to reserve the above-mentioned date for the above-mentioned event. Full payment of the booking fee shall be made to Nanyang Pho Leng Hui Kuan at least two (2) weeks prior to the event.

Indemnity

I, representing my organisation, hereby undertake that in the usage of Nanyang Pho Leng Hui Kuan facilities for my event, shall not hold Nanyang Pho Leng Hui Kuan, its staff or its authorised agents responsible or in any way liable for any damage, loss of any personal property, personal injury, mishap, accident and/or death whatsoever that may arise directly or indirectly as a result of or in connection with the usage.

By submitting this form, I hereby consent to allow Nanyang Pho Leng Hui Kuan (NPLHK) to collect, use, disclose and/or process my personal data in order to process, administer, facilitate, maintain and/or manage my relationship with NPLHK as a client, event organiser (“Purpose”), including communications on NPLHK’ activities, programmes, events, carrying out research, analysis and development activities for NPLHK’ purposes; and making disclosures required by law or a competent authority. NPLHK may, for the above Purpose, disclose my personal data to its third-party service providers and/or agents, which may be sited outside of Singapore (subject always to requirements under applicable law having been met).
